Yep, that 2 page document provides the description of the process required
to validly execute the Living Will, etc. documents.  I did not include the
Living Will documents themselves.  Just like what you found on the hospital
association site, there are several flavors of template docs out there. My
lawyer said the version on the Colotado BAR site is very widely used with
minimal modifications.
